Remember to refer to mail and fax deadlines when determining conference registration fees.

To insure that participants receive full value, program administrators reserve the right to decline registrations.  In the event that registration is declined, any program fees paid will be promptly refunded.

Cancellation Policy: A written cancellation postmarked no later than March 31, 2009, entitles you to a refund, less a $100 processing fee.  After that date, no refunds are issued.  You are encouraged to send a substitute if you cannot attend.

VA Attendee room rate: $110.00-single or double from April 21 – 24, 2009, based on availability and is subject to change. All VA reservations must be made via the
VA housing form
. You must present a credit card at the time of check in for room and incidental charges. The deadline for all VA reservations is March 19, 2009.

Department of Defense and Veterans Administration personnel will be required to present a valid photo ID indicating VA or DOD status at hotel registration to receive the government hotel rate.

Because rooms at the VA rate are limited, unfortunately we cannot guarantee the VA rate to everyone who submits a housing form. You will receive a confirmation email from SHCA once you fill out the required form. If you do not receive a confirmation email within three business days of submitting the form, please call staff at (312) 422-3700 to avoid disappointment at check-in.

Department of Veterans Affairs Patient Advocates and Customer Service attendees are invited to join Joan Van Riper and Teresa Ard for discussion of Responses on SHEP Complaint Process Questions, Post Discharge Calling Contact Performance Measure, Integration of Patient Advocate Data with other Quality Management Data, Reports for Management, PATS and an informal open discussion time.   

This meeting will take place in conjunction with the SHCA Annual Conference on Tuesday, April 21, 2009 from 3:00 p.m. – 5:30 p.m.
